Student Records

The office of the registrar houses academic records for current and former (PAL) Charter Academy students.
The Registrar can assist you with:

  • Transcript requests (a student’s signature & ID Verification is required with each request)
  • Verification of diplomas conferred to former students
  • Verification of dates of attendance
  • Verification of enrollment
  • Authorization for release of grade, account, and academic information to your parents/guardians after Graduation

The information above (except as noted for transcript requests) is student directory information and may be released without the student’s consent. However, students may file a request to prevent the release of their student directory information.

Obtaining Transcripts

The transcript request process requires a signature and a copy of your valid I.D./Driver’s License.
You must complete a request form and return it to the Office of the Registrar by fax, e-mail, traditional mail or in person.

All outstanding balances with the school must be paid before transcripts will be processed.
The transcript request fee is $5 per official transcript.

Delivery methods: Mail, Fax, in Person Pick-up
Monday requests will be completed by Tuesday 12 noon
Tuesday requests will be completed by Thursday 12 noon
Wednesday requests will be completed by Thursday 12 noon
Thursday requests will be completed by Tuesday 12 noon

*$5.00 Payment Per Transcript. Fees are to be paid at time of request.
Cash or Money Order accepted.
*Copy of Identification Card/Drivers License (REQUIRED)
*Transcript Request Form:
         · Current name and past name
         · Date of birth
         · Dates of attendance
         · Current mailing address
         · Home phone number
         · Address where transcript is to be sent
         · Number of copies

*For transfer students: The high school the student is transfering to can REQUEST BY FAX OR EMAIL a request for an Official Transcript. The school must include the student’s name, date of birth, and last semester attended.

Transcripts or Records

Cost for unofficial transcript:
Free

Accept faxed transcript request:
-Yes, for unofficial transcripts only. (Unless requested by transfer high school) If student is faxing a request he/she must include: Request Form and Valid ID

-Faxed requests are accepted for official transcripts, however transcripts will not be released without Request Form, copy of valid photo ID, and mailed or hand delivered payment.

Accept e-mail/internet request:
-Online requests are accepted for unofficial transcripts only*. Please use the online form below. You must include all required information. 

*Email/internet requests CAN BE SUBMITTED for official transcripts, however transcripts will not be released without mailed or hand delivered payment and copy of the student’s valid ID. 

REPLACEMENT DIPLOMA

Cost for Replacement Diploma: $25.00

Diplomas take 6-8 weeks, and payment Per Diploma is to be paid at time of request. Cash or Money Order accepted.
Records Request must be completed before purchase.
